How can I apply for a new DIS book?

Single holder demat accounts: 

Application Method	Process
Online on MyGeojit	If AMC dues (if any) are cleared, you can place a DIS book request on MyGeojit.  For more information, refer our article on how to apply through MyGeojit.
Offline through a branch	* In case your old DIS book has been phased out (as per NSDL Circular: 2016/0059) as part of DIS standardisation, you can either:     i. Send an email from your registered email ID with your request, OR     ii. Submit a DIS Book Request Form at a Geojit branch * The request will be processed on payment of the applicable charges at a branch.

Offline through a branch- In case you have used up your DIS book	* Submit the DIS requisition form (at the back of the booklet) at a Geojit branch. * The request will be processed on payment of the applicable charges at a branch.
Offline through a branch- In case you have lost your DIS book	* Submit the following at a Geojit branch:     i. DIS Book Request Form     ii. A request letter for the cancellation of the         existing DIS book and issuance of a new         DIS book.     iii. ID proof of all Demat account holders     iv. Transaction Statement (Non-DCN clients           only) * The request will be processed on payment of the applicable charges at a branch.
Offline through a branch- In case your old DIS book has been phased out (as per NSDL Circular: 2016/0059) as part of DIS standardisation	* Submit a DIS Book Request Form at a Geojit branch * The request will be processed on payment of the applicable charges at a branch.

The payment for physical and email requests will have to be made at a branch.
